Integrated Social Worker

Full Time 35 hours a week

Kitsumkalum is seeking an ambitious person for the Integrated Social Worker position. The Integrated Social Worker is primarily responsible for supporting, developing and maintaining a range of family care and support.

Qualifications and Educational Requirements

  • Bachelor’s degree in Social Work or Social Work Certificate from an approved school plus one (1) year recent related clinical social work experience in a healthcare
  • Basic First Aid
  • Updated Criminal Record

Knowledge, Skills, and Abilities

  • First Nations Culture and traditions
  • Provides clinical counseling services to patients and families and identified problems, personal concerns and treatment objectives; consults and collaborates with the interdisciplinary teams regarding patient care issues, resources and the discharge planning
  • Monitor clients, document provide verbal and written reports on observations
  • Prepare and submit monthly/yearly reports and other paperwork in a timely manner
  • Liaise and coordinate services with mental health, addictions, and other medical professionals and act as an advocate for clients
  • Plan, promote, and participate in home visits, case conference, workshops, and community events. to increase outreach and awareness in our communities
  • Knowledge of resources available to Aboriginal clients and knowledge of the referral process for other agencies
  • Knowledge of First Nation Health Authority Patient Travel Policy and Procedure
  • Knowledge and experience in providing culturally safe and competent care, specifically pertaining to Aboriginal
  • Excellent oral and written communication skills
  • Computer skills using Microsoft Office suite (MS Word, Excel)

Working/Special Conditions

  • May be required to work evenings and weekends
  • Exposure to regular travel to semi remote community and winter road conditions
  • Use of own vehicle with business insurance is required
  • Must possess and maintain a valid class 5 driver’s licence
