First Nations Health Authority are surveying community members and health leaders to gather input on health programs and services and opportunities for improvement. This includes programs and services funded or delivered by the FNHA as well as those delivered by health system partners.
The results of the evaluations will support decision-making, continuous learning, improvements at the FNHA, and partner efforts in learning, growing and maturing their relationship to advance shared goals.
At the end of the survey, you may choose to provide your contact information for an opportunity to enter random monthly draws to win a $50 gift card. All survey participants are eligible to enter this draw. Please complete the survey before the end of September 2024.
