PCN Traditional Wellness Coordinator

JOB SUMMARY: PCN Coordinator and Traditional Wellness Provider
In partnership with the BC Ministry of Health, Northern Health Authority, and Pacific Northwest Division of Family Practice, Kitsumkalum Health Centre is excited to bring primary care providers directly to our community, to ensure every member has the opportunity to access physician-led team-based care.
Reporting to the Kitsumkalum Health Director and supported through the regional Primary Care Network (PCN) team, the PCN Coordinator and Traditional Wellness Provider ensures Kitsumkalum context is considered within all areas of PCN development, coordination, implementation and evaluation, while providing administrative supports to the PCN and healthcare teams.
The PCN coordinator and traditional wellness provider acts as the primary point of contact for the integration of PCN within the context of the cultural practices, protocols, values and beliefs of the Kitsumkalum First Nation, community members, families and individuals. Following the vision set by the community, the PCN Steering Committee and the service plan for the PCN, the ideal candidate brings a wealth of cultural knowledge, administrative and coordination skills to the team. The position requires the ability to work well under pressure, follow directions and be self motivated in managing assigned tasks and all related activities.

TYPICAL DUTIES & RESPONSIBILITIES:
- Provides cultural and administrative support to the Kitsumkalum implementation and healthcare team and other committees as assigned on projects and initiatives
- Acts as the primary resource to ensure alignment of PCN and culturally safe work, within the context of Kitsumkalum
- Supports a place-based approach in the development of the PCN implementation workplans, coordination of project activities and tracking of progress
- Coordinates and facilitates community, health service and stakeholder engagement activities
- Plans, coordinates and supports cultural training and traditional wellness activities with healthcare staff, PCN Steering Committee, groups, other committees as requested, as well as community members
- Brings in traditional expertise to support service implementation, delivery and evaluation
- Organizes and facilitates meetings, works with partners on agenda setting; prepares and distributes meeting materials; identifies, tracks and follows up on action items
- Provides reports on PCN implementation activities and outcomes, and communities progress of the PCN as directed by the Kitsumkalum Health Director
- Develops cultural and administrative resources for team members and the public, including resource pathways, manuals, communication briefs and emails
- Collaborates with colleagues to streamline and optimize operational processes and procedures
- Works closely with the PCN evaluation team to coordinate culturally safe, place-based PCN data collection. This includes gathering data to meet PCN mandatory requirements
- Performs other duties as required
EDUCATION, TRAINING AND EXPERIENCE:
- Preferred background in healthcare, Health Information Management, Administration assistance, or a related field and 3-5 years of experience or an equivalent combination of education and experience
- Knowledge of the Tsimshian language and culture is an asset
- Current valid B.C. driver’s license, reliable transportation and willingness to submit a driver’s abstract
- Current criminal records check including vulnerable sector is required
SKILLS AND ABILITIES:
- An understanding of the impact of colonial systems on the health and wellbeing of Indigenous individuals, families and communities
- Experience working in Indigenous health, wellbeing, culture and/or engagement
- Experience managing complex projects from conception through to evaluation
- Experience with gathering data, data collation, and communicating complex information
- Experience in community engagement, change management or action planning
- Skilled at collating information from diverse team members to prepare briefing notes, background information, agendas and minutes
- Ability to remain flexible in the midst of evolving priorities
- Committed to the ongoing development of competencies in cultural safety, reciprocity and reconciliation
- A passion for improving community health and wellbeing while supporting service provider satisfaction
- Proficiency in Office software including Word, Excel, Outlook and Powerpoint
