Community Health Nurse [CHN]

Full time: Monday – Friday 8:00 am -12:00 pm 1:00 pm – 4:00 pm

Wage Range: $72,800.00 – $ 81,900.00

Reporting to: The Community Health Nurse reports directly to the Kitsumkalum Health Director.

Position Summary: The Community Health Nurse [CHN] works with Kitsumkalum Health staff to provides health promotion services that promote the prevention of disease. The CHN initiates treatment plans including health care management and therapeutic interventions. In accordance with BC nursing statutory and regulatory standards.

The CHN collaborates with patients, families and allied health professionals to identify a care plan that supports the needs of the client. Develop and implements population and evidence-based strategies to improve health and wellness in the community.

The CHN participates in peer review and self-review to evaluate the quality of outcomes of services at the patient, client, resident, community and population level. Working within the context of the client-centered, family focused. in accordance with CRNBC Standards of Practice, the CHN combines her/his health sciences knowledge with assessment, supervisory and clinical nursing skills to effectively assess and coordinate client care. The CHN provides direction, guidance and supervision of the Home Support Workers.

Qualifications:

o Member in good standing of the College of Registered Nurses of British Columbia (CRNBC)

o Preferred qualifications Baccalaureate degree in nursing from a Canadian University or recognized equivalent

o Experience in public health or community-based setting

o Valid Class 5 Driver’s License and own vehicle

o Complete Criminal Record Check

Knowledge, Skills, and Abilities:

o Knowledge and understanding of First Nations communities, health and social issues

o Knowledge and understanding of the population health approach and the social determinants of health

o Excellent written and interpersonal communication skills and the ability to work effectively with a variety of people and circumstances

o Ability to teach and facilitate learning

o Ability to guide and supervise Home Support Workers and the ability to establish realistic performance expectations

o Self-directed with demonstrated organizational skills

o Tolerance and sensitivity to the needs of every individual and their right to be treated with respect and fairness

o Appropriate verbal and non-verbal communication patterns, behaviours, skills and knowledge

o Ability to maintain a professional demeanour and confidentiality

Responsibilities:

o Participates in the planning, development and evaluation of the Kitsumkalum Community Health Program

o Coordinate home care services in such a way to meet the home care service needs of the clients while encouraging independence, family and community support

o Receives referrals to assess client need for Home and Community Care Services and support

o Home and Kysen Hospital Visits

o Completes a structured client assessment that includes ongoing re-assessment and determines client need and service allocation

o Develops a client centered care plan implement the care plan to provide in-home personal care services in collaboration with the Health staff to support home and hospital visits

o Provides mentoring, direction, guidance, support and supervision for Kitsumkalum Home Support Workers

o Provides in-home direct nursing care as required

o Documents the assessments, care plans and service delivery on the clients personal Home Care Health Record, Monthly, Yearly Reports and Yearly Work Plans

o Completes and submits service delivery data and reports as required by FNHA Home and Community Care Program and Kitsumkalum Health Centre reports

o Adheres to infection-control guidelines

o Maintains client confidentiality and ensures that all records are stored securely

o Provides clients and family caregivers with health information as required through demonstrations and informal teaching sessions

o Assess the need for capacity building and conducts in-service training sessions for the Home Support Workers and other members of the health team as appropriate

o Initiates, facilitates and participates in case management and family conferences as appropriate

o Maintains nursing skills through attendance at workshops, conferences, in-service and staff meeting, in addition to reading current literature and Best Practice Guidelines

o Ensures that clients admitted to the Home and Community Care Program have access to medical supplies and equipment

o Maintains professional support and consultation to ensure the program is delivered in a safe effective manner

o Establishes internal and external linkages with other professionals and community resources

o Ability to communicate effectively both verbally and in writing

o Ability to deal with others effectively

o Physical ability to carry out the duties of the position

o Ability to utilize a variety of computer software applications.
